1. Birth Certificate / Certificatdo de Nacimiento
2. Proof of Residency (utility bill / lease) / Prueba de domicilio (cuenta de agua o luz / contrato de renta)
3. Immunization Records / Registro de vacunas 
4. Social Security Card / Tarjeta de Seguro Social
5. Income Verification / Comprobante de ingresos
6. SNAP-TANF-Medicaid Eligibility Form (Not card) / SNAP-TANF-Medicaid  Carta de beneficios (No Tarjeta)
7. Parent - Guardian ID / Identificación del padre o tutor
