PLEASE CALL US AT 972-502-1700 TO SET UP AN APPOINTMENT AND HELP YOU COMPLETE THE REGISTRATION. PLEASE BRING WITH YOU THE FOLLOWING DOCUMENTS

  1. Birth Certificate 
  2. Proof of Residency (utility bill / lease) 
  3. Immunization Records  
  4. Social Security Card 
  5. Income Verification 
  6. SNAP-TANF-Medicaid Eligibility Form (Not card) 
  7. Parent - Guardian ID
